By accessing or using the ANX Home Healthcare and ANX Hospice Care (“Service”), you agree to be bound by these Terms and Conditions. If you do not agree, do not use the Service.
2. Description of Service
ANX Home Healthcare and ANX Hospice Care provides a web-based intake referral management platform for home health and hospice care organizations. The Service facilitates patient referral coordination, status tracking, notifications, and care plan management across multiple organizational units.
3. SMS/Text Messaging Program
As part of the Service, ANX Home Healthcare and ANX Hospice Care offers an SMS notification program (“SMS Program”) to keep patients, family members, and healthcare partners informed about referral status.

- Description: Automated text messages providing referral status updates, appointment reminders, and care coordination alerts.
- Message Frequency: Message frequency varies based on referral activity. Typically 2–10 messages per referral lifecycle.
- Message and Data Rates: Standard message and data rates from your wireless carrier may apply.
- Opt-Out: Text STOP to any message to unsubscribe. You will receive a confirmation message and no further texts.

- Supported Carriers: Compatible with all major U.S. carriers including AT&T, Verizon, T-Mobile, Sprint, and others.
- No Purchase Required: Consent to receive SMS messages is not a condition of purchasing any goods or services.
4. User Accounts
Access to the Service requires an authorized account. You are responsible for maintaining the confidentiality of your login credentials and for all activities that occur under your account. You must notify us immediately of any unauthorized use.
5. Acceptable Use
You agree not to:
- Use the Service for any unlawful purpose.
- Attempt to gain unauthorized access to any part of the Service.
- Share login credentials with unauthorized individuals.
- Transmit any malicious code or interfere with the Service’s operation.
- Use the Service to access, store, or transmit PHI in a manner that violates HIPAA.
6. Protected Health Information
The Service handles Protected Health Information (PHI) as defined by HIPAA. All PHI is encrypted at rest using AES-256-GCM and in transit using TLS. Access is controlled through role-based permissions and all access is logged for audit purposes.
7. Intellectual Property
All content, features, and functionality of the Service — including software, text, graphics, logos, and design — are the exclusive property of ANX Home Healthcare and ANX Hospice Care and are protected by copyright, trademark, and other intellectual property laws.
8. Limitation of Liability
To the maximum extent permitted by law, ANX Home Healthcare and ANX Hospice Care shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the Service. The Service is provided as a coordination tool and does not constitute medical advice.
9. Service Availability
We strive to maintain continuous availability of the Service but do not guarantee uninterrupted access. Scheduled maintenance, updates, or circumstances beyond our control may result in temporary unavailability.
10. Termination
We may suspend or terminate your access to the Service at any time for violation of these Terms or for any other reason at our discretion. Upon termination, your right to use the Service ceases immediately.
11. Changes to Terms
We reserve the right to modify these Terms at any time. Changes will be posted on this page with an updated date. Continued use of the Service after changes constitutes acceptance.
12. Governing Law
These Terms are governed by the laws of the State of California, without regard to conflict of law principles.
