To get started with the process, all we need are:

• Physician’s order and/or verbal order
• Current insurance information
• Current Medication List and other pertinent medical information
• Patient’s name, address and phone number

Our Home Health services will initiate within 48 hours from referral date or on date specified by the physician

On admission

• Our staff will collaborate admission with all physicians involved in the patient’s care within 24 hours.
• A Plan of Care (POC) will be completed within 48 hours and sent out to the physicians’ office for approval and signature. The main components of the Plan of Care (POC) are the following but not limited to:
  • Diagnosis
  • Updated Medication List including allergies
  • Physicians’ Order for Discipline and Treatments including amount, frequency and duration of treatment. (i.e. Nursing, P.T.)
  • Goals / Rehab Potential and Discharge Plans

During Care

• If there are significant changes in patients’ condition, Clinical Track Reports will be sent out for updates.
• If our patient is transferred to the hospital or any other care facility, we will send Transfer Summary Form to communicate latest medical information.
• If our patient needs care after 60 days; a 60 Day Summary Form is sent to the ordering physician.

Before Discharge

• • If our patient met all the goals set initially or during care, or if our patient does not have any further “skilled needs” we will advise you or your office, patient, family, caregivers and / or significant others involved in the care of our plan for discharge.
  • Discharge Summary is available upon your request.
