ANX Nurses

ANX Hospice Care provides both hospice care and palliative care, delivered by an interdisciplinary team of healthcare professionals. We serve San Francisco and San Mateo counties, with future expansion to Santa Clara County. All current job openings will be based out of our Daly City corporate headquarters.

RN Case Manager

Job summary: To provide quality nursing care to terminally ill Hospice patients, as a member of the Interdisciplinary Team (IDT), in a safe and effective manner.  RN determines patient’s initial and ongoing eligibility for hospice services.  Provides assistance and an understanding of patient/family home care situation and in time of bereavement.  

Qualifications:  

  • Minimum 1 year experience in hospice care in the past 3 years, preferred
  • Graduate from an accredited school of nursing and current licensed as a Registered Nurse to practice in CA
  • Must be available on rotating on-call/weekend schedule with visit capability

Medical Social Worker

Job Summary: MSW is a core member of the Interdisciplinary Team in the provision of varied social work services to Hospice patients and their families.  Provides initial and  ongoing emotional, spiritual, psychosocial assessments, counseling, bereavement services, and community education, outreach and referral.

Qualifications:

  • Masters of Social Work (MSW) degree from school of social work accredited by the Council on Social Work Education.
  • Minimum 2 years of experience as a social worker, preferred.
  • Active patient contact within the past 3 years, preferred.
  • Must be available on rotating on-call/weekend schedule with visit capability

Licensed Vocational Nurse (LVN)

Job Summary:  Works under the supervision of RN in the provision of quality patient care.  Responsible in direct patient care, following the plan of care in providing treatments.  Works collaboratively with the members of the Interdisciplinary Team meeting patient End of Life goals.

Qualifications:

  • Graduate from an accredited practical nurse or vocational nursing program
  • Current LVN license with minimum 1 year experience
  • IV/Blood draw certification preferred
  • Must be available on rotating on-call/weekend schedule with visit capability

Hospice Aides

Job Summary:  Supervised by the Registered Nurse in providing essential personal care and assistance to the terminally ill hospice patients, per plan of care. Provides emotional support and compassion care to patients and family.

Qualifications:

  • Current certification to practice as Home Health Aide in CA
  • Minimum 2 years of experience as a nursing assistant, and active patient contact within 3 years, preferred
  • Must meet federal Home Health Aide training and competency requirements
  • Demonstrates and maintains hospice competency, experience and/or training
  • Must be available on rotating on-call/weekend schedule with visit capability

Volunteers

Volunteers are integral part of the hospice team – they fill the gap between patients, their loved ones, and their care team. Volunteer services include Pet Care, Music or Art therapies, and simple Companionship. In addition to pharmaceutical pain management, these alternative services can help with overall palliation.

All hospice volunteers receive free and comprehensive training before being assigned tasks that align with their skills and interests. You will learn hospice philosophy, caring for the terminally ill, grief and loss education, health and safety precautions and more. You will work with a clinical team consisting of a doctor, nurse, aide, chaplain, and social worker.

Qualifications

  • Volunteers must be at least 18 years old. High school students may be allowed upon approval of the Administrator/Director.
  • All prospective volunteer candidates are required to submit to a background test.
  • To ensure that volunteers are equipped with the challenge of working with the dying, they are required to complete extensive orientation and training sessions.
  • Most volunteers are required to have a valid driver’s license especially if the visit to patient homes require traveling.
