Our skilled nurses hold Advanced Cardiac Life Support Certifications and are Board Certified Wound Care Specialists, so you can be assured of the highest quality care.

We offer intermittent physician-ordered skilled nursing and other health care services for patients who are homebound due to illness or injury. Before we make any recommendations, we first conduct an in-depth interview and needs assessment with the referring physician and patient in order to determine the best course of action and develop a customized plan of care. We understand that each patient has a unique set of needs and preferences.

ANX Home Healthcare provides the following services:

• SKILLED NURSING – comprehensive wound and ostomy care, pain management, diabetes management, cardiac rehabilitation including blood pressure, anticoagulant medication management, and leg edema prevention and management, secondary to chronic venous insufficiency.
• PHYSICAL THERAPY – home exercise program, balance/coordination training, post-surgical rehabilitation, orthotic training, and instruction in use of assistive devices.
• OCCUPATIONAL THERAPY – training and restoration of dexterity required for daily physical function, including fine motor skills, orthotics/splinting and use of adaptive equipment
• HOME HEALTH AIDE – personal care and daily living assistance
• MEDICAL SOCIAL WORKER – coordinate patient access to community resources for counseling, including individual and family therapy, as well as determining needs and requirements for alternative living arrangements such as assisted living and nursing facilities.
• REGISTERED DIETITIAN – nutritional assessment, counseling and education; development of customized dietary regimens with sensitive attention to patient’s cultural/religious constraints.
• SPEECH AND LANGUAGE THERAPY – treatment and therapy for voice disorders, speech intelligibility, auditory comprehension, cognitive ability, functional communication, swallowing/oral-motor skills and verbal fluency.
